City of Guelph R-Zone Incident Reporting Form

This form is required to be completed and submitted to the SSHA league executive and the City of Guelph staff anytime an incident occurs that is contrary to the R-Zone policy.

Since that time we have been providing a fun, nurturing, positive hockey experience for kids aged 4 to 17 in the city of Guelph, Ontario. We offer a structured hockey league that is focused on skill development, team play, and most of all fun.
